You will be eligible the first day of the month following thirty (30) days of full-time employment. You must enroll within thirty (30) days of eligibility. The following is a summary, this is not a contract of benefit coverage and any and all may change at any time.
Health Insurance, administered by Cigna, offers four levels of coverage with employee premium starting at $20.82 monthly. If you enroll in health insurance the company provides you with $20,000 in life insurance including HSA option. Health, Dental and Vision insurance is available to all new employees the first of the month following the date of hire.
Vision Insurance, administered by VSP, provides an annual exam with a $10.00 in-network co-pay Out-of-network benefits are also available. Premium for employee-only coverage is $1.00 per month.
Dental Insurance, administered by MetLife provides in-network benefits at 100% preventative, 80% basic and 50% restorative up to $1,000/$1,500 in annual expense after a $50.00 deductible. Out-of-network benefits are also available. Premium for employee-only coverage is $3.00 per month.
Long Term Disability Insurance pays a benefit of 60% of salary if you are disabled due to accident or illness lasting longer than 90 days. LTD is administered by The Hartford. Mile Hi pays the total cost of LTD insurance.
Short Term Disability Insurance pays a benefit of 60% of salary if you are disabled due to accident or illness lasting up to 90 days; there is a two week waiting period. STD insurance is administered by The Hartford. The costs are paid by you.
401(k) participation is available the first of the month after 90 days of employment. You may defer (pre-tax) up to the IRS maximum ($16,500 annually). The company matches 50 cents per your dollar up to 4% of your earnings. The Plan includes a Roth option.
Paid Sick Leave, employees earn one (1) hour of paid leave per thirty (30) hours worked (“accrued leave”), up to 48 hours per year.
Vacation entitlement occurs after each anniversary: 1 year = 1 weeks; 2 years = 2 weeks; 5 years – 3 weeks; 12 or more years 4 weeks.
FSA Plan, pay for unreimbursed health expenses with a pre-tax account available to enroll when eligible for all other benefits.
Mile Hi observes six (6) holidays a year for all employees. Your schedule may require you to work; if so, you will be paid for the holiday plus any time that you work.
Jury Duty pay is regular wages for up to five days.
Bereavement pay is two (2) paid days.
